Advanced Practice Practitioners (Luke Fisk, FNP & Melynda Myers Mallory, FNP): $29/month
Patients 17 years and under with a participating parent $25/month
Jonathan Vellinga, MD: $59/month
Erik Lundquist, MD: $99/month
Premium Functional Medicine Membership Plan
Includes everything in the Basic Plan, plus enhanced access and benefits:

âś“ Dedicated phone, text, and chat line for quicker responses during business hours
âś“ 10% discount on supplements.
âś“ Receive insights on genetic testing and other epigenetic biological age tests.
For Dr. Lundquist’s Premium Members:
âś“ Priority in-person visits when he is in the office
âś“ Telemedicine acute appointments available Monday–Friday when he is not in office

Non-membership patients are assigned to a designated TCIM provider and do not receive the enhanced access or additional benefits included with our Basic and Premium Membership plans. Copays, deductibles, coinsurance, and other patient responsibility amounts may apply based on your individual insurance coverage.
